Dear Parents,

 

                                                KINDERGARTEN REGISTRATION

 

            Kindergarten registration will be held at Memorial School, Dyer Avenue on the following days.

                                    Tuesday          April 20            1:30-   3:00  p.m.

                                    Wednesday     April 21            1:30-   3:00  p.m.

                                    Thursday         April 22            1:30-   3:00  p.m.

 

            Kindergarten registration is only for children who are not already attending Memorial School.

 

            A child registering for Kindergarten must be five years old on or before October 1, 2010.

 

Please bring the following or your child will not be registered:

 

1.      Proof of residency (e.g. current tax bill – if you are a homeowner or if you are renting a notarized landlord affidavit and Certificate of Inspection (issued by the town). 

2.   Your child's original birth certificate or Passport (if not a U.S. Citizen).

            3.   Immunization records with verification from doctor.

            4.   Medical reports if medical problems exists.

            5. Physical Exam form (attached) – also available on our school website                                               www.shmemorial.org) completed by your doctor within 365 days of September start                       date.

                 

PLEASE NOTE: Your child will not be registered without the above information.

 

The Speech-Language Specialist will individually screen all Kindergarten children. Speech-Language screening time is approximately 10-15 minutes for each child at the time of registration.

 

NO CHILD WILL BE PERMITTED TO ENTER SCHOOL WITHOUT A PHYSICAL EXAMINATION OR WITHOUT FULFILLMENT OF ALL IMMUNIZATION REQUIREMENTS.

 

 

 

 

 

 

                                                                             

 

CHILD FIND

 

 

            If you have or know of a pre-school age child with a physical, mental or emotional difficulty, our school's "CHILD FIND" can help . . . with a free screening and with a special educational program.

 

            Have the child's parents contact the school today.  If we can find these youngsters, we can help them.  The person to contact is Miss Tricia Smith, Memorial School, Dyer Avenue, South Hackensack, New Jersey, 201-440-2965, during the hours of 8:30 a.m. to 3:30 p.m., Monday through Friday.
